Indiana Secondary Transition Resource Center-Transition Matrix 
The Transition Assessment Matrix was developed for secondary education transition teachers by the Indiana Northeast Cadre of Transition Leaders and the Indiana Secondary Transition Resource Center (INSTRC).  Assessments included in the Matrix have either been created by INSTRC, are available online, or are those for which INSTRC has received permission from the original publisher.  Recent Matrix updates make it easier to navigate and identify assessments that meet the specific needs of your students.

University of Oklahoma-Zarrow Center
The Zarrow Center facilitates student-directed educational, employment and adult living outcomes for individuals with disabilities, fosters innovative self-determination oriented instruction and transition education practices, and prepares educational leaders.

 

The Texas College and Career Readiness Support Center is a collection of resources and tools available to all stakeholders committed to building a strong college and career readiness culture in Texas schools. Housed at Education Service Center Region 13, the center’s two main projects, the Texas College and Career Readiness Profile Planning Guide and the Texas Online College and Career Readiness Resource Center, were created to support all educators as they continue to assist students for successful post-secondary options. The center’s website hosts these major state-funded initiatives for easy access and reference.
 

San Antonio Community College District (ACCD)
The Alamo Colleges District serves the Bexar County community through its programs and services that help students succeed in acquiring the knowledge and skills needed in today's world. Students are taught by highly qualified faculty with Master's and doctorate degrees dedicated to creating a learning centered environment. Student services include counseling, computer labs, tutoring, financial services, services for the disabled, developmental instruction, veteran's services, and job placement.

Texas A&M San Antonio-TU CASA 
The TU CASA model is designed to allow students with intellectual disabilities to receive a true university experience while gaining the skills and knowledge needed to transition to independent adult lives. TU CASA addresses the critical needs in the community by establishing a sustainable program that: (a) provides 18 months of fully inclusive PSE opportunities with access to typical and specialized A&M-SA services and supports; (b) offers a meaningful credential upon completion that addresses academic, employment, and independent living domains; (c) utilizes a person-centered approach to meet unique student needs and enhance their quality of life; (d) employs comprehensive program evaluation; and (e) includes a plan for replication on regional and state levels.

Palo Alto-Project ACCESS
Project ACCESS began in  2016, at  Palo Alto College.  Offering unique post-secondary educational access to individuals with intellectual disabilities, and it's the first program of its kind in San Antonio. Project Access, enables students with intellectual disabilities take college-level courses to eventually earn their General Office Level 1 Certificate and acquire necessary skills for gainful employment.

Bobcat RISE (Resilience, Independence, Self-Determination, Empowerment) is a student-centered program embodying Texas State University's commitment to excellence and innovation in teaching, research, creative expression, and service. RISE expands the breadth of institutional inclusion by welcoming non-traditional students with intellectual and developmental disabilities (ID/DD) and empowering them to develop to their highest potential, govern their own lives, and participate fully and freely in the community as citizens of Texas, the nation, and the world.

With a commitment to equity and excellence, Think College supports evidence-based and student-centered research and practice by generating and sharing knowledge, guiding institutional change, informing public policy, and engaging with students, professionals and families.
 

National Center on Secondary Education and Transition (NCSET)
The National Center on Secondary Education and Transition (NCSET) was originally funded by the U.S. Department of Education's Office of Special Education Programs from 2000-2008, during which time it partnered with six major organizations also focused on the secondary education and transition of youth with disabilities. NCSET coordinated national resources, offered technical assistance, and disseminated information related to secondary education and transition for youth with disabilities in order to create opportunities for youth to achieve successful futures.

An organization with a high level of professionalism for VACs , Transition Specialists, Job Coaches, Vocational Counselors, Life Skills Teachers and other professionals working directly with VAC students or in the area of transition.

Texas Workforce Solutions Vocational Rehabilitation Services serves youth and students with disabilities to help prepare for post-secondary education and employment opportunities.  Services are eligibility and need based. 

The Texas Department of Aging and Disability Services (DADS) was created to administer long-term services and supports for people who are aging as well as for people with intellectual and physical disabilities.

The YouthRules! initiative promotes positive and safe work experiences for teens by providing information about protections for young workers to youth, parents, employers, and educators. Through the YouthRules! initiative, the U.S. Department of Labor and its partners promote positive and safe work experiences that help prepare young workers in the 21st-century workforce.

Child Labor Laws cover any employee under 18 years of age. Once an individual reaches age 18, they are considered an adult under child labor laws. The Texas Child Labor Law ensures that a child is not employed in an occupation or manner that is harmful to the child's safety, health or well-being.

The Bureau of Labor Statistics of the U.S. Department of Labor is the principal Federal agency responsible for measuring labor market activity, working conditions, and price changes in the economy.

TAC §75.1023 Provisions for Individuals Who Are Members of Special Populations (Career and Technical Education)

The purpose of the project is to provide and maintain an updated web-based platform to assist stakeholders such as local educational agencies (LEAs), advocates, and parents in learning about state and federal laws and regulations by providing rich resources regarding obligations and rights for the provision of a free and appropriate public education in the least restrictive environment to students with disabilities.
This assessment tool was developed for Autism Speaks through a contract with Virginia Commonwealth University’s Rehabilitation Research and Training Center.
